Split images directory into subdirectories, like attachments

Description by paul:
Images are all stored in one big directory right now. Accessing the files in this directory will start to get really slow once the number of files gets between 10,000-20,000. This should be broken up into a series of smaller directories, each holding about 1000 files each. This is similar to how the attachments work.
